We are a social enterprise organisation driven by our vision and values not by profit.

Our vision is to work closely with GPs and local communities in Cambridgeshire and Peterborough to help them strengthen general practice and community nursing for the future and manage the rapidly rising demand for health care by

• delivering community nursing services for adults in their own homes in a truly patient-centred and holistic way, promoting self-care and independence,
• providing a range of nurse led clinics that deliver good clinical outcomes for patients and value for money and
• delivering services to support the development, recruitment and retention of highly competent general practice and community nurses

We put patients at the centre of their own health care, empowering them to have the confidence, ability and the skills to achieve the best outcomes possible. We believe that supporting each individual to work towards self-management through education, encouragement, goal setting, sensitive challenge, when appropriate, and collaborative problem solving is the key to achieving successful outcomes.

We aspire to provide a challenging, rewarding and enjoyable work environment that inspires nurses to relish the ‘art of nursing’ and to be the best they can be. With excellence in patient care at the heart of what we do, we will recruit and retain a highly motivated and capable workforce.

Our team are DBS checked, our policies and protocols are evidence based and our services are audited to ensure our outcomes are excellent and patient experience is good.

Our team are well trained, experienced and able to effectively problem solve with patients, their families and colleagues.

Our team work collaboratively with patients, their families and each other to achieve mutually agreed goals.

Our team treat patients, their families and each other with dignity, respect and thoughtfulness.
